I find it most difficult to keep the up/down pattern going solidly when I have a down stroke on a string and then it should be followed by an up stroke on the next string above it (for example, a down stroke on the D string followed by an up stroke on the A string). It seems a little easier to just do two down strokes in a row there, but then of course there's an interruption in the alternate picking, which feels a little awkward, too. What do most accomplished pickers do in that situation?

Always DUDUDU. If you play Down then another Down when moving to another string it will have a tendency to throw your rhythm out of kilter. So Down on the beat, Up off the beat. I'll be interested in any answers to your question. I have tried, unsuccessfully, for years to learn how to cross pick and find it just doesn't work with the down up down up when going from string to string. I end up hitting the wrong string(s) and it throws my rhythm off. I've also tried the down down up and that works for me sometimes.

It's weird and hard to do for the picking hand, you are right.
But I would stick to the rule and practice same exercises addressing just that.
The way you angle the pick is important too, when you play the downstroke tilt the pick a little towards the low strings.
